Scott Hanson, DO

Scott Hanson, DO, CMD is a Board-Certified Physician in Family Medicine with certification in Geriatric Medicine. He graduated from The University of Northern Iowa and then attended medical school at Des Moines University. He completed a Family Medicine Residency Program through the University of Minnesota in Mankato, Minnesota, and then completed a fellowship in Geriatrics at The University of Nebraska Medical Center in Omaha, Nebraska. He has also received certification as a CMD (Certified Medical Director) through the American Board of Post-Acute and Long-Term Care Medicine.

He has been working in Long-Term Care since his graduation from fellowship in 2016. He is a member of the American Geriatrics Society, AMDA, American Board of Family Medicine, the Iowa Health Care Association, and The Iowa Society for Post-Acute and Long-Term Care Medicine. Scott lives in Winterset, Iowa, with his wife and three sons. He enjoys running, BBQ, and spending time with his family.

Lindsay Miklo, DNP

Lindsay Miklo is a Geriatric, Doctoral Prepared Nurse Practitioner in the Des Moines Metro. She has been in health care since 2010 with a special interest in older adult care. The life stories of the young-at-heart adult population are priceless to her. She strives to promote functional recovery and provide continued care while meeting the unique healthcare needs of aging adults.

She completed her undergraduate work with a Bachelor’s Degree in Biology from The University of Iowa and a Bachelors in Nursing from Grand View University before earning her Doctor of Nursing Practice from the University of Michigan, Flint. She has multiple years of experience as a primary care provider in Long-Term, Assisted Living, and Post-Acute Care Facilities. In her free time, she enjoys spending time with her family including two active boys, traveling, and following The Iowa Hawkeyes. She is thankful for the opportunity to partner with patients and families in providing personalized care for the older adult.

Mary Dennis, NP-C

Mary Dennis is a Board-Certified Adult-Gerontology Nurse Practitioner. Her education includes graduating from DMACC in 2000 with her ADN Degree, Kaplan University with a Bachelor of Science in Nursing Degree in 2013, and a Master’s Degree in Nursing Practice in 2016, also from Kaplan University.

She is a member of the American Academy of Nurse Practitioners. Mary started working with Long-Term Care patients in 1991 as a CNA. She has 16 years of nursing experience in Long-Term Care, Subacute Care, and Assisted Living along with 5 years of experience as a Long-Term Care Nurse Practitioner. She enjoys making connections with residents and families; assisting with managing chronic disease and improving quality of life. She is experienced in working with a multidisciplinary team to achieve this. She has also worked closely with managing chronic wounds and hospice for end-of-life care.

Outside of work, she resides in Ankeny, Iowa, and has 2 adult children. She enjoys gardening, biking, reading, baseball, classic rock concerts, and traveling.

Millennium Therapy specializes in customized, individualized care at Calvin Community. As an Iowa-based company, Millennium is committed to the health and wellness of our community. Millennium Therapy provides physical, occupational, and speech therapy in a variety of clinical settings including Skilled Nursing, Assisted Living, and Independent Living here on campus. Our full-service therapy programs are designed to maximize our resident’s independence, function, and safety.

Dr. Jana Poock

Dr. Jana Poock has been practicing in the Des Moines area for over a decade. She is a native of the Des Moines area. Dr. Poock received her Bachelor’s Degree in Biology and Religion from Wartburg College in Waverly, Iowa. She then attended Scholl College of Podiatric Medicine in Chicago, Illinois where she obtained her Doctorate in Podiatric Medicine. She completed her education in St. Louis, Missouri where she did a 2-year surgical residency at DePaul Health Center and served as Chief Resident during her final year. She enjoys all aspects of podiatric medicine and would be happy to assist you with all of your foot care needs.
